RESOLUTION AUTHORIZING THE: A) PRESIDENT OF THE BOARD TO EXECUTE A FIRST AMENDMENT TO THE AGREEMENT WITH JEFFERSON UNION HIGH SCHOOL DISTRICT C/O DALY CITY YOUTH HEALTH CENTER FOR THE PROVISION OF HIGH SCHOOL BEHAVIORAL HEALTH CAREER PATHWAYS PROJECT INCREASING THE MAXIMUM OBLIGATION BY $11,721 TO A NEW MAXIMUM OF $106,721, WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2010 THROUGH JUNE 30, 2011; AND B) CHIEF OF THE HEALTH SYSTEM OR DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on August 30, 2010, the County Manager approved an Agreement with Jefferson Union High School District c/o Daly City Youth Health Center for the provision of mental health services and outpatient mental health services to youth, for the term of July 1, 2010 through June 30, 2011, for a maximum obligation of $95,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a First Amendment to the Agreement, to increase the maximum obligation by $11,721 to a new maximum of $106,721 with no change to the Agreement term; and

WHEREAS, this Board of Supervisors has been presented with a form of the First Amendment and has examined and approved it as to both form and content and desires to enter into this First Amendment to the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said First Amendment on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Chief of the Health System or designee be authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
