RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE THE SECOND AMENDMENT TO EACH OF THE AGREEMENTS WITH CATHOLIC CHARITIES CYO; CITY OF SOUTH SAN FRANCISCO; AND PENINSULA VOLUNTEERS, INC., INCREASING THE COLLECTIVE AMOUNT BY $141,600 FOR FY 2011-12 FOR A TOTAL OF $424,800 WITH NO CHANGE TO THE TERM OF JULY 1, 2009 THROUGH JUNE 30, 2013

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 9, 2009, your Board adopted Resolution 70153 approving the Agreements with Catholic Charities CYO; City of South San Francisco Adult Day Care Program; and Peninsula Volunteers, Inc., for the provision of adult day care services in the amount of $141,600 for the term July 1, 2009 through June 30, 2013; and

WHEREAS, on May 11, 2010, your Board adopted Resolution 70754 approving Amendment One to the Agreements increasing the collective amount by $141,600 for the period July 1, 2010 through June 30, 2011 for a total obligation of $283,200 with no change to the term; and

WHEREAS, the parties now wish to amend the Agreements by increasing the collective amount by $141,600 for FY 2011-12 changing the total obligation to $424,800 for the purpose of continuing adult day care services for the term July 1, 2009 through June 30, 2013; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, the Second Amendment to the Agreements between the County of San Mateo and Catholic Charities CYO; City of South San Francisco; and Peninsula Volunteers, Inc., reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Second Amendment to the Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
