RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH CREEKSIDE MENTAL HEALTH REHABILITATION PROGRAM FOR THE PROVISION OF MENTAL HEALTH REHABILITATION AND LONG TERM CARE SERVICES DECREASING THE MAXIMUM OBLIGATION BY $204,930 TO A NEW MAXIMUM OF $682,860, WITH NO CHANGE TO THE TERM OF JULY 1, 2009 THROUGH JUNE 30, 2012

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 9, 2009, this Board approved an Agreement with Creekside Mental Health Rehabilitation Program, by Resolution 70155, for the provision of mental heath rehabilitation and long-term care services, for a maximum obligation of $739,825; and

 

WHEREAS, on December 1, 2009, this Board approved an Amendment to the Agreement with Creekside Mental Health Rehabilitation Program, by Resolution 70531, for the provision of mental health rehabilitation and long-term care services, to increase the maximum obligation by $147,965 to a new maximum of $887,790 with no change to the term of the Agreement; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Second Amendment to the Agreement, reference to which is hereby made for further particulars, to decrease the maximum obligation decrease the maximum obligation by $204,930, to a new maximum of $682,860 with no change to the Agreement term; and

 

WHEREAS, the Board of Supervisors has examined and approved the Second Amendment to the Agreement as to form and content and desires to enter into the Second Amendment to the Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
