BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE: A) PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT FIVE TO THE AGREEMENT WITH AMERICAN MEDICAL RESPONSE WEST FOR ASSESSMENT AND TRANSPORT OF PERSONS WITH BEHAVIORAL EMERGENCIES AND FOR THE PRESCHEDULED TRANSPORT OF BEHAVIORAL HEALTH AND RECOVERY SERVICES AND AGING AND ADULT SERVICES CLIENTS FOR COURT APPEARANCES, EXTENDING THE TERM FROM APRIL 30, 2010 THROUGH JUNE 30, 2011, AND INCREASING THE MAXIMUM OBLIGATION BY $62,240 TO A NEW MAXIMUM OBLIGATION OF $2,008,440; AND B) ACCEPTANCE OF A DONATION FROM THE POLICE CHIEFS AND SHERIFFS ASSOCIATION OF $117,000 TO THE HEALTH SYSTEM TO HELP FUND THE AGREEMENT WITH AMERICAN MEDICAL RESPONSE WEST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the parties entered into an Agreement on December 7, 2005, for the assessment and transport of patients with behavioral emergencies and for the prescheduled transport of Behavioral Health and Recovery Services and Aging and Adult Services clients for court appearances; and

 

WHEREAS, the Agreement was amended on February 10, 2009, to increase the maximum obligation by $162,000 for a maximum obligation of $1,296,000 and extend the term through June 30, 2009; and

 

WHEREAS, the Agreement was amended on July 14, 2009, to increase the maximum obligation by $81,000 for a maximum obligation of $1,377,000, and extend the term through September 30, 2009; and

 

WHEREAS, the Agreement was amended on September 29, 2009, to increase the maximum obligation by $258,000 for a maximum obligation $1,635,000, and extend the term through June 30, 2010; and

 

WHEREAS, the Agreement was amended on September 14, 2010, to increase the maximum obligation by $311,200 for a maximum obligation $1,946,200, and extend the term through April 30, 2011; and

 

WHEREAS, the parties now wish to amend the Agreement to increase the maximum obligation by $62,240 for a new maximum obligation of $2,008,440 and extend the term through June 30, 2011.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute Amendment Five to the Agreement for and behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED to accept a donation of $117,000 to the Health System from the Police Chiefs and Sheriffs Association to help fund the Agreement.
