RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A FOURTH AMENDMENT TO THE AGREEMENT WITH FREE AT LAST FOR THE PROVISION OF ALCOHOL AND OTHER DRUG SERVICES, INCREASING THE MAXIMUM OBLIGATION BY $25,000 TO $5,098,962, WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2009 THROUGH JUNE 30, 2011

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 9, 2009, this Board approved an Agreement with Free At Last, for the provision of alcohol and drug services, and authorizing the Chief of the Health System or designee to execute contract amendments which modify the funding amount by no more than $25,000 in aggregate, for the term of July 1, 2009 through June 30, 2011, for a maximum obligation of $5,047,580; and

 

WHEREAS, on April 13, 2010, this Board approved a First Amendment to the Agreement to incorporate Second Chance Act Re-Entry Residential Services, increasing the maximum obligation by $81,600, for a new maximum obligation of $5,129,180, with no change to the Agreement term; and

 

WHEREAS, on May 18, 2010, the Chief of the Health System approved a Second Amendment to the Agreement to increase the Bay Area Service Network funding in the amount of $6,577, for a new maximum obligation of $5,135,757, with no change to the Agreement term; and

 
 

WHEREAS, on November 2, 2010, this Board of Supervisors approved a Third

Amendment to the Agreement whereby to reduce Net County Funding by $24,795, decrease funds paid in FY 08/09; decrease Second Chance Act Re-Entry Allocation funds for residential services by $61,200; increase Second Chance Act Re-Entry outpatient and intensive day treatment services by $24,200; and reduce the administrative fee to the First Offender Program from 10% to 8%, for a new maximum obligation of $5,073,962, with no change to the Agreement term.

 

WHEREAS, there has been presented to this Board of Supervisors a Fourth Amendment to the Agreement whereby to increase Second Chance Act Re-Entry Residential Services by $25,000 to a new maximum obligation of $5,098,962, with no change to the Agreement term.

 

WHEREAS, this Board has examined the Fourth Amendment to the Agreement and has approved it as to both form and content and desires to enter into the Amended Agreement.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Fourth Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
