RESOLUTION AUTHORIZING THE A) PRESIDENT OF THE BOARD TO EXECUTE A THREE –YEAR AGREEMENT, WITH KAISER ON-THE-JOB TO CONDUCT PRE-PLACEMENT AND OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION (OSHA) MANDATED PHYISCAL EXAMINATIONS, FITNESS FOR DUTY AND RETURN TO WORK EXAMINATIONS FOR THE TERM OF JANUARY 1, 2011 THROUGH DECEMBER 31, 2013 NOT TO EXCEED $660,000 B) HUMAN RESOURCES DIRECTOR OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the Board of Supervisors wishes to enter into an agreement with an occupational health clinic to manage the County’s Occupational Health Programs on behalf of the County;

 

WHEREAS, the Director of Human Resources recommends that the County execute a three year agreement with Kaiser On-the-Job for the County’s Occupational Health Programs from January 1, 2011 to December 31, 2013;

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration the agreement and the Board has approved it as to both form and content and desires to enter such agreement; and

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreement with Kaiser On-the-Job not to exceed $660,000 for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

    BE IT FURTHER RESOLVED that the Human Resources Director or the Director's designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
