ADOPT A RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT ONE TO AGREEMENT EPO P3-41 WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH FOR THE HEALTH SYSTEM TO PROVIDE PUBLIC HEALTH EMERGENCY PREPAREDNESS SERVICES, AMENDING THE TERM OF THE AGREEMENT TO JULY 31, 2009 THROUGH JULY 30, 2011, AND INCREASING THE AGREEMENT MAXIMUM BY $100,000 TO A NEW MAXIMUM OF $1,284,499

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the parties entered into Agreement EPO P3-41 on December 15, 2009, for the Health System to provide public health emergency preparedness services for a maximum obligation of $1,184,499 for the term of July 31, 2009 through July 30, 2010; and

WHEREAS, the parties now wish to amend the Agreement to extend the term from July 31, 2009 through July 30, 2011, and increase the maximum obligation by $100,000 for a new maximum obligation of $1,284,499.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute Amendment One to the Agreement for and behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
