RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A FIRST AMENDMENT TO THE AGREEMENT WITH MATEO LODGE, INC. FOR THE PROVISION OF RESIDENTIAL AND MENTAL HEALTH TREATMENT SERVICES AND MANAGEMENT OF THE LINDEN STREET FIRE, INCREASING THE MAXIMUM OBLIGATION BY $30,000 TO A NEW MAXIMUM OF $1,873,469 WITH NO CHANGE TO THE AGREEMENT TERM OF JULY 1, 2010 THROUGH JUNE 30, 2011

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 8, 2010, this Board approved an Agreement with Mateo Lodge, Inc., by Resolution 070819, for the provision of residential and other mental health treatment services for the term of July 1, 2010 through June 30, 2011, for a total obligation of $1,843,469; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a First Amendment to increase the maximum obligation by $30,000, to reimburse Contractor for out-of-pocket expenses incurred from the Linden Street fire, to a new maximum of $1,873,469 with no change to the Agreement term; and

WHEREAS, this Board has examined and approved this First Amendment as to both form and content and desires to enter into the amended Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said First Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
