Welcome to the HEAR2T Program at San Mateo County!



The County's Nurse Health Coaching Program for heart disease and diabetes is modeled after the highly successful and nationally recognized Stanford Heart2 Program.

San Mateo County was a company partner in the Stanford Health Network for almost 10 years. Since 2005, the county has continued the Heart2 and Diabetes Program under the direction of the Employees Wellness Program. The Nurse Health Coach is an independent contractor. This insures that the program is confidential.

This program is a free, confidential service. To be eligible for health coaching you must have one or more cardiovascular or diabetes health risks. You may be referred to the Nurse Health Coach as a result of your participation in the County's Wellness Screening Program or you can self-refer.
