Since this was dropped off erroneously at Health Dept & fees are wrong (PAD & CDP public hearing fees due); I've left message for applicant saying that if fees aren't paid by 8/22/05, I'll CLOSE case & return all materials to him.

Applicant's daughter came in on 8/22/05, spoke with Farhad, who didn't know that I had the PLN file. Thus he sent her up to Env Health to retrieve the voided check. She had the Env Info form but didn't submit it for same reason. I called applicant (Burdette) this AM & told him to send his daughter in again, that I have the file, & to submit the correct fees (we'll return the voided check) & ENV form. At that point I'll assign project.

4/24/07 LAA - This project is appealable to the Coastal Commission. Not in appeals jurisdiction, but a domestic well is not a principally permitted use in the PAD, so appealable to CC per Section 6328.3(s) & (q). 2/1/06 MAT: Although not appealable to CCC, still needs public hearing at ZHO because of PAD permit. 12/5/5 MAT: Printed activity report for Greg Ward at counter. Told him delay was due to need for ZHO public hearing. After checking CCC appeals area maps, determined this did not need ZHO hearing - it is a staff-level CDP.

Set for hearing at 5/3/07 ZHO, but will recommned that the item be continued, since during review of staff report, the following issues arose & need to be resolved: 1. Clarification of distance from well site relative to pond or reservoir on property. 2. Clarification if it is a "pond" or a "reservoir". Env. Health requirements for distance to well are different for each. 3. Clarification of soil type at location of well site (not clear if "lands suitable for ag" or "prime ag"). 4. Implications of approval of this CDP/PAD request vis a vis the Wm Act Contract on the parcel. 5. Clarification of private access easement location - is it the same as route that currently provides access to well site via informal roads/trails? 6. Response from applicant regarding suggestion from Env. Health to propose multiple well sites under this permit, in case first well is unproductive.
